At 2L’s Rx, we are committed to protecting the privacy, confidentiality, and security of your personal health information. This policy outlines how we handle your data in compliance with the Personal Health Information Protection Act (PHIPA), 2004.

We collect personal health information directly from you or from the person acting on your behalf. The information we collect may include:

Health History: Medical history, current medications, previous injuries, and physical assessment notes.
Treatment Records: Clinical notes regarding your progress and the treatments provided.
Financial Info: Insurance details and billing history for services rendered.
2. Purpose of Collection (Why we need your data)
We collect and use your information for the following clinical and administrative purposes:
To provide safe and effective Clinical Massage and Remedial Exercise treatments.
To develop and maintain a treatment plan tailored to your Length & Longevity goals.
To communicate with other members of your Circle of Care (e.g., your GP or Physiotherapist) to ensure coordinated treatment.
To process billings with health insurance providers.
To comply with the regulatory requirements of the College of Massage Therapists of Ontario (CMTO).
3. Data Storage and Security
Your digital records are stored securely using Jane App, a PIPEDA/PHIPA-compliant practice management platform. All data is encrypted and stored on secure servers located within Canada. Any physical records at our 80 King St. W. location are kept in a double-locked, secure environment accessible only by authorized clinical staff.
4. Retention of Records
In accordance with Ontario provincial regulations, we are required to retain your health records for at least ten (10) years from the date of your last visit. If the patient is a minor, records are kept for ten years after the patient turns 18. After this period, records are destroyed in a secure and confidential manner.
5. Your Rights and Access
You have the right to:
Access: Request a copy of your personal health record.
Correction: Request a correction to any information you believe is inaccurate or incomplete.
Withdraw Consent: You may withdraw your consent for the collection or use of your data at any time (subject to legal or contractual restrictions).
Note: A reasonable administrative fee may apply for the preparation and delivery of copies of your clinical files.
